037 - WHOLESALER’S MONTHLY BEER REPORT. This form is used by licensed beer wholesalers in Maryland to report their monthly acquisitions of beer from suppliers. The form is a mandatory filing for all authorized wholesalers who distribute beer to retail dealers within the state and must be submitted to the Comptroller of Maryland by the 10th day of the month following the reporting period. It requires the licensee to list each supplier’s name and Maryland license number, along with the total gallons of beer received from each during the month. This report must be filed even if no transactions occurred during the reporting period. In cases where corrections are needed to a previously submitted report, the form can be marked as an amended return. The report must be signed by an authorized representative of the business, such as an owner, partner, or corporate officer, certifying its accuracy and completeness.
