FA-609 - NOTICE OF HEARING AND MOTION TO ENFORCE PHYSICAL PLACEMENT ORDER. This form is used in Wisconsin Circuit Court when a parent or legal party is requesting judicial enforcement of a court-ordered physical placement schedule for a child. It is filed in cases involving either marriage or paternity and is used when one party believes the other has violated the terms of an existing court order regarding physical placement. The motion may allege that the moving party was denied court-ordered time with the child, that such time was substantially interfered with, or that the other party failed to exercise their placement without adequate notice, causing financial loss. The form requests the court to take corrective action, which may include awarding additional placement time, reimbursing costs, holding the other party in contempt, or modifying the placement schedule. A hearing is scheduled, and the other party must be personally served with notice at least five business days in advance.
