CR-261 - VERIFICATION OF TIME SERVED. This is a Wisconsin Circuit Court form used to document the exact amount of confinement time an inmate has served on a specific count for purposes of sentence adjustment. The form records the original sentence length, initial confinement and extended supervision terms, any subsequent confinement imposed, and adjustments due to disciplinary “bad time.” It also identifies whether the inmate is serving additional sentences and requires supporting judgments of conviction when applicable.
