010-1 - ORDER FOR ADDITIONAL VEHICLE IDENTIFICATION CARDS BY HOLDER OF A PUBLIC TRANSPORTATION; PUBLIC STORAGE & TRANSPORTATION; OR INDIVIDUAL TRANSPORTATION PERMIT. This form is used by existing permit holders to request extra vehicle identification cards for additional vehicles under their current permit. This form is not intended for original permit applications but is solely for ordering identification cards for vehicles added after a permit has already been issued. Applicants must provide their existing permit number, name, and contact details as they appear on the permit. The form requires specifying the number of additional vehicles needing identification and must be submitted with a $10.00 fee for each vehicle ID requested.
