LF-27 - APPLICATION TO WITHDRAW UNCLAIMED FUNDS. This form is used in bankruptcy cases filed in the United States Bankruptcy Court for the Southern District of Florida to request release of funds that were deposited with the court as unclaimed funds under 28 U.S.C. §§ 2041 and 2042. This form allows an individual, debtor, business entity, authorized representative, attorney, or funds locator to assert entitlement to the funds and request that the Clerk remit them. The application requires detailed claimant information, identification of the funds, proof of authority or entitlement, government-issued identification, and certification that no competing claims exist.
