Description

SELF REPRESENTED LITIGANTS (SRL) COMPLAINT FOR ADMINISTRATIVE REVIEW. These forms are used in the Circuit Court of DeKalb County, Illinois, as part of a Self-Represented Litigant (SRL) Complaint for Administrative Review process. The purpose of the SRL Complaint for Administrative Review is to allow an individual, without an attorney, to challenge a final decision issued by the Illinois Department of Employment Security (IDES), including the Director of the Department and the Board of Review. The complaint seeks judicial review under the Illinois Administrative Review Law (735 ILCS 5/3-101 through 5/3-112), typically after the denial of unemployment benefits or similar employment-related decisions. The plaintiff must allege that the agency’s decision is legally incorrect and attach a copy of the decision being contested. Accompanying this complaint is the Summons in Administrative Review, which formally notifies the defendants (IDES, its Director, the Board of Review, and the employer involved in the original agency proceedings) that the complaint has been filed and that they are required to respond within 35 days. The summons includes detailed instructions on where and how to file an answer, including information about the mandatory e-filing system. The form also includes a Certificate of Mailing section to document service of the summons by certified mail, and a Sheriff’s Return section to record personal service attempts, including details of when, where, and to whom the documents were delivered. www.FormsWorkflow.com
