Description

CVDC05 - STIPULATION TO USE OF ALTERNATIVE DISPUTE RESOLUTION PROCESS (CRC 201.9). This is a San Luis Obispo County Superior Court form used by parties to agree to resolve a civil case through an ADR method instead of trial. The form allows selection of options such as mediation, neutral case evaluation, binding or nonbinding arbitration, or other agreed processes. It identifies the chosen neutral and confirms that all parties and attorneys consent to the ADR process. Once approved by the court, the stipulation becomes a court order governing how the case will proceed. The form may also include directives regarding case status, including limitations on adding parties and dismissal of unserved claims. Optional. SLO Local Rule 9.11 4. www.FormsWorkflow.com
