Physicians Report

PHYSICIAN’S REPORT (ADULT WARD). This form is used in the Circuit Court of the Eighth Judicial Circuit in Florida for guardianship or guardian advocacy cases. It is completed by a licensed physician who has recently examined the adult ward. The form provides critical information to the court regarding the ward’s mental and physical condition, the purpose and date of the medical examination, and the ward’s capacity to live independently. It also includes the physician’s professional assessment of whether the ward continues to require the assistance of a guardian and whether the ward is currently capable of being restored to full capacity.
