TF-304 PA - AUDIO RECORDING REQUEST. This form is used to request audio recordings of court proceedings held at the Palmer Courthouse through the Alaska Court System. To obtain a copy of a hearing's audio, a requestor must complete and submit the form either in person, by mail, or via email. The form must include specific case information such as the case number, hearing date, proceeding type, media number (CD, tape, or digital), and precise log numbers, which identify the start and end times of the desired recording. These details are found in the case's log notes, which can be reviewed at the courthouse or requested separately. Once the request is submitted, the court issues an invoice. Upon payment, the audio is delivered by the selected method: mail, in-person pickup, agency box, or electronically through ZendTo. Audio for court use (such as exhibits) must be provided on a physical CD. Only parties to a case may request confidential recordings, and those individuals must present valid photo identification and complete a sworn statement before a court clerk or notary. Confidential case types include adoption, guardianship, delinquency, and others specified by law. Depending on when the hearing occurred, the recording format may vary—cassette, FTR CD, or CourtSmart CD. Mac users are advised to specify compatibility needs.
