Form 6-2 - AFFIRMATION AND DESIGNATION FOR SERVICE OF PROCESS. This form is used in proceedings within the Family Court of the State of New York for the appointment of a guardian or standby guardian of the person of a minor. The form serves two primary functions. The affirmation section requires the proposed guardian or standby guardian to formally declare that they are over the age of 18, are eligible to serve in the role, and that they will faithfully, honestly, and lawfully perform the duties of guardian or standby guardian of the child. The designation for service of process section authorizes the Clerk of the Court in the relevant county to accept legal documents on behalf of the guardian if the guardian cannot be located within New York State after reasonable efforts have been made.
