DLC 4120 - APPLICATION TO TRANSFER A LIQUOR PERMIT(S). This application IS required when a permit holder is transferring its liquor permit to another: • Owner (whether person or business) at the same location where the permit is currently issued (TRFO); • Owner at a different address location (TFOL); or • Location where the current owner will continue to operate the permit (TRFL). For your application to be deemed complete, you MUST fully and legibly: • Answer and sign all required sections and questions (“*” indicates a required field); • Pay all the required fees listed below; and • Submit any additional required listed forms. This specific application is NOT required for the following changes/transactions: • If the name of the permit holder remains the same and all that is changing is the underlying ownership (i.e. ABC, Inc remains the permit holder, but the shareholders are changing). Here, you MUST complete and mail or drop-off in person, as applicable, the Transfer of Membership Interest in an LLC Application or the Change in Corporate Stock Ownership Application. • For Items A through D below, email the requested documents (write permit number on each) to the Division at liquordocs@com.ohio.gov: a. Court appointed Receiver (e.g., bankruptcy, etc.), Executor to an Estate, or Guardian – submit copy of certified Court Order; b. Change of name due to marriage or divorce, including restoration of former name – Submit copy of certified court order; c. Converting the business type (e.g., Inc to an LLC) – Submit applicable entity disclosure forms and Ohio Secretary of State documents showing the name change or conversion. Note that the Secretary of State Charter # should remain the same; d. Changing LOCATION ONLY for an H permit or a S-1, S-2, B-2A permit that does NOT have an issued Ohio A Class permit issued with it.
