PL-CV903 - ANNUAL NOTICE OF CONSERVATEE’S RIGHTS. A conservatorship has been established for you; a conservatorship is when a judge appoints another person to act or make decisions related to personal care or financial affairs of a person who is either physically or mentally unable to handle either or both. The conservator is the person the judge appoints, and the conservatee is the person the court determines to be unable to protect and manage their own personal care and/or financial affairs. A conservatorship can only be established if other less restrictive options won't work. In conservatorships, the court investigator conducts field investigations and assessments with individuals applying for a conservatorship of the person and/or estate. When gathering information, the investigator interviews involved parties, relatives, attorneys, medical and psychiatric staff, various government agencies, and other concerned parties. The investigator obtains statements, affidavits, and other evidence to prepare detailed reports and make recommendations to the judicial officer regarding the suitability of proposed conservator. If a conservatorship is established, the investigator may also review the continued need for the conservatorship. Probate Code 1835.5 requires that the court provide conservatees with a list of their rights within the conservatorship, which rights the court withheld, and contact information for the conservator(s), Court Investigator, and the person to end or change the conservatorship.
