Description

CARE-105 - ORDER FOR CARE ACT REPORT. This court order form is used to initiate CARE Act proceedings in the Superior Court of California. It confirms that the court has reviewed a petition (form CARE-100) submitted by a petitioner requesting the commencement of CARE Act proceedings for a respondent. The court finds that the petition shows prima facie evidence that the respondent is or may be eligible for participation in the CARE Act process. The court orders a county agency or its representative to engage with the respondent and submit a written report by a specified date. This report should include information about the respondent's residence, eligibility for the CARE Act, efforts to engage the respondent, conclusions on their ability to engage in services voluntarily, and other relevant details. The court also requires the county agency to use a specific form (CARE-106) to serve notice of this order to the petitioner, respondent, and respondent's counsel as per California Rules of Court. Additionally, the court has appointed an attorney to represent the respondent in these CARE Act proceedings, with their contact details provided. The form includes the date and the name of the judicial officer. Form Adopted for Mandatory Use Judicial Council of California. Welfare & Institutions Code, § 5977(a)(3). www.FormsWorkflow.com
