MV-15F - FREEDOM OF INFORMATION LAW REQUEST FORM. This form is used in the Department of Motor Vehicles (DMV) to request access to certain DMV records in the state of New York. The form requires you to provide your personal information, describe the records you are requesting, and select a payment method.
