State of Minnesota County Judicial District: Court File Number: Case Type: District Court Adoption ­ Stepparent In Re the Petition of: , Petitioner/Parent and , Petitioner/Stepparent To Adopt: , (child's current name) , (child's current name) , (child's current name) Pursuant to Minnesota Statues § 259.53, subd. 5, and based upon the attached Petitioners' Affidavit in Support of Ex Parte Motion to Waive Post-Placement Assessment, Petitioners request that the Court waive the requirement of a Post-Placement Assessment and Report and also request that the Court schedule a final hearing on the Petition for Stepparent Adoption. Petitioners understand that even if the Court waives the Post-Placement Assessment, the background study and fingerprint check must still be completed. In lieu of a Post-Placement Assessment Report, attached is Petitioners' Stepparent Affidavit Checklist. In the alternative, if the Court declines to waive the Post-Placement Assessment and Report, Petitioners request that the Court direct that the (name of county where adoption is taking place) County Adoption Unit conduct the same type of PostPlacement Assessment, at the same cost to Petitioners, as is conducted when a stepparent seeks to adopt a child and the marriage is of less than one year's duration. Petitioners' motion is based upon the attached Affidavit in Support of Motion to Waive PostPlacement Assessment and upon all of the files and records herein. Enclosed is a proposed order waiving post-placement assessment.
