Description
AFFIDAVIT OF HEIRS (§ 732.103, Fla. Stat.). This form is used in the probate division of the Circuit Court of the Eighth Judicial Circuit in Florida to identify and list all relatives of the deceased individual, including spouses, children, grandchildren, parents, siblings, nephews/nieces, and grandparents. The form requires providing the name, age, address, and if applicable, the date of death for each relative. It also includes sections for indicating if the spouse had children who were not also children of the deceased, if there are no surviving spouse or children, and if any deceased children had their own children (grandchildren of the decedent). Additionally, the form requires information about the parents of the decedent, siblings of the decedent, children of deceased siblings (nephews/nieces), and grandparents of the decedent. There is also a section for including information about kindred of the last deceased spouse if applicable.
