MC 218 - ORDER REVOKING RELEASE AND FORFEITING BOND, NOTICE OF INTENT TO ENTER JUDGMENT. This form is used in Michigan's Judicial District and Circuit Courts when a defendant or juvenile who was released on bail fails to comply with the conditions of their release, such as not appearing in court or violating other terms of their release. The form specifies the violations, the type of bond posted, and the full bail amount.
