That the Plaintiff obtained a judgment against the Defendant(s) on the ____ day of ____________________, 20____ for the sum of $______________. $____________ plus interest of __________________ remains unpaid on the judgment. 2. That the Plaintiff does not believe that the Defendant(s) has/have in his/her possession visible property upon which a levy can be made sufficient to satisfy the judgment. 3. That the Plaintiff has reason to believe that the above named Garnishee, has in its hands, possession or control, goods, monies, chattels, or effects belonging to the Defendant(s). 4. That the property or thing sought to be garnished is not (1) wages for personal labor or services due the head of a family residing in the State of Florida, (2) the cash surrender value of life insurance policies issued upon the lives of citizens or residents of the State of Florida, (3) disability income benefits due under any policy or contract of life, health, accident or other insurance.
