SDSC PR-121 - FINANCIAL STATEMENT COVERSHEET. This is a probate form used in the Superior Court of California, County of San Diego, to accompany financial statements submitted with an accounting in conservatorship, guardianship, trust, or estate matters. The coversheet identifies the applicable accounting period and confirms that supporting financial statements are attached. In guardianship and conservatorship cases, the form also includes a verification confirming that electronic account statements were received and printed without alteration. Optional Form. Prob.
