FORM TMAPPL - APPLICATION FOR REGISTRATION TRADEMARK OR SERVICE MARK. This is a Georgia Secretary of State Corporations Division form used by an individual or business entity to register a trademark or service mark for use within the State of Georgia. Filed pursuant to O.C.G.A. §§ 10-1-440 et seq., this application places the mark on public record and provides statewide notice of the applicant’s claim of ownership. The form requires the applicant’s identifying information, a description of the mark, the goods or services associated with the mark, classification number, dates of first use (anywhere and in Georgia), and the manner in which the mark is used in commerce. Applicants must submit required specimens, a drawing page of the mark, and a $15 non-refundable filing fee.
