CASE NO.: DEPT: ORDER APPOINTING TEMPORARY GUARDIAN(S) OVER MINOR DATE OF EXPIRATION: UPON REVIEW of the verified Petition for Appointment of Guardian(s) and Declaration submitted by the Petitioners, the same having been reviewed by the Court, and there being good cause to believe that a temporary guardianship is in the best interest of the minor child: THE COURT FINDS that the proposed protected minor child, (child222s name) , born on (date of birth) , faces a substantial and immediate risk of financial loss or physical harm and/or needs immediate medical attention or other necessities of life and will not be afforded such attention or necessities unless this temporary guardianship is issued.
