CIV 3 - DECLARATION IN SUPPORT OF ENTRY OF SATISFACTION OF JUDGMENT (Small Claims). This is a Kings County Superior Court form used in small claims cases when a judgment debtor seeks court recognition that a judgment has been paid. The form allows the debtor to declare that full or partial payment has been made and to request that the court enter a satisfaction of judgment. It includes sections to identify the payment amount, date, and recipient, and to explain situations where the judgment creditor has not filed an acknowledgment, cannot be located, or is deceased.
