CV033 - ADMINISTRATIVE APPEAL – GENERAL PROOF OF SERVICE Government Code § 53069.4(b)(1). This form is used in the Superior Court of California, County of Marin, to document that a Notice of Administrative Appeal has been properly served on the appropriate government agency. The form is completed by a person over the age of 18 who is not a party to the case and records the method of service, including personal delivery or first-class mail. It also identifies the agency served, the date and time of service, and the address where documents were delivered.
