MOD-13 - DECLARATION OF RESTRAINED PERSON RE: FIREARMS. This form is used in the Superior Court of California, County of Modoc, in cases involving restraining orders. The form requires the restrained person to declare whether they currently possess any firearms and to acknowledge their legal obligations under California law. If the restrained person has firearms, they must confirm understanding of the requirement to surrender or sell them within a specified timeframe and to file proof of compliance with the court.
