He has executed a written form to waive notice, deny his paternity, relinquish the child for adoption, or consent to the adoption of the child. His whereabouts or identity are unknown or cannot be ascertained. He has been served with written notice of his alleged paternity and the proposed adoption, and he has failed to bring an action pursuant to Fam. Code § 7630(c) within 30 days of service of the notice or the birth of the child, whichever is later. Notice to the above-named alleged father has been given, as required by Fam. Code § 7666. The above-named father is not the presumed father of the child. The San Diego County Health and Human Services Agency has filed a report, as required by Fam. Code § 7663. It is in the best interest of the child to terminate the parental rights of the above-named alleged father so that the adoption of the child may proceed. The consent of _________________________________________ is not necessary for the adoption of the child.
