TADR-03 - ALTERNATIVE DISPUTE RESOLUTION STATUS REPORT (ADR). This is a mandatory form used by the Superior Court of California, County of Fresno, to provide an update on the status and outcome of an Alternative Dispute Resolution (ADR) process such as mediation, arbitration, or neutral case evaluation. The form requires identifying information for the case—including attorney or party names, contact details, case type, amount in controversy, and date of the complaint filing. It also records the date and facilitator of the ADR conference, the type of resolution process used, and whether the case was resolved as a direct or indirect result of ADR. If the case settled, the form reminds parties that a Notice of Settlement or Dismissal must be filed with the clerk. It further asks parties to estimate attorney and expert fees saved or additional costs incurred due to ADR participation, as well as the number of court days saved or added by using the process.
