Name: Address: City, State and Zip: IT APPEARING to the Court that the Respondent herein is currently unempl oyed and that Child Support payments have not been forthcoming. IT IS ORDERED that said Respondent shall actively and diligently seek em ployment including, but not limited to: 1. Going to the Illinois Job Service once a week and following up on al l leads; 2. Applying to at least five (5) new places each week for jobs which he/she can perform; 3. Keeping a list of places and dates where applications have been file d, verified in writing by a representative of the firm; 4. Reading the Help Wanted section of all local newspapers, responding to appropriate ads and keeping copies thereof; 5. Not refusing any job which he/she is physically able to do, even if the job is not in his/her chosen trade or occupation, or is not full-time, or pays less than the pay he/she is use d to earning, or is temporary; and, 6. Upon procurement of employment, the paying parent may be required to sign an Income Assignment which will be held in the file of the Court by the Circuit Clerk until the Court order s otherwise; 7. Payments must be made in U.S. dollars, check, bank/postal money orde r or certified check. Gifts, clothes, food or money paid directly to the children or to the other parent will not be c ounted as part of the child support. 8. Visitation problems and questions relating to custody are no excuse for non-payment. If they are a problem, the parent must come to the Court to settle the problem and Support must be p aid until the Court orders differently. 9. This cause is continued to at in Room of the Kane County Judicial Center, 37W777 Rt. 38, St. Charles, IL, for status at which time Respondent shall be present with written proof of compliance with the above provisions.
