SOS/BCII 8016 - REQUEST FOR LIVE SCAN SERVICE. This is a California Secretary of State form used by individuals applying for or renewing a Notary Public commission who must complete a fingerprint-based criminal background check through the Live Scan system. The form provides the information needed by the California Department of Justice and, when applicable, the FBI to process fingerprint submissions and return criminal history results to the Secretary of State. Applicants must provide personal identifying information, acknowledge receipt of privacy notices, and present the completed form to an authorized Live Scan operator. The form also includes required state and federal privacy disclosures, applicant rights information, and instructions regarding the collection, use, and review of criminal history records.
