JD-CL-54 - REQUEST AND STIPULATION FOR REFERRAL TO A PRIVATE ALTERNATIVE DISPUTE RESOLUTION (ADR) PROVIDER. This is a Connecticut Superior Court form used when all parties agree to submit their case to a private ADR provider. The form allows parties to jointly request mediation, arbitration, or another ADR process, identify the selected provider, define the scope of issues to be resolved, and specify the requested referral period.
