This form is issued by the California Fair Political Practices Commission (FPPC) and is used by certain auditors, claims managers/adjusters, and other designated employees as specified in their agency’s conflict of interest code (Regulation 18733). Unlike the standard Form 700, this form requires disclosure only if the filer was assigned to or participated in a case involving an economic interest in which they had a financial interest.
