JD-JM-134 - SCHOOL VIOLENCE PREVENTION PROGRAM MOTION, ORDER, DISPOSITION. This is a Connecticut Superior Court form used in juvenile delinquency cases involving alleged acts or threats of violence on school property or at school-sponsored activities. The form allows a child to apply for placement in a court-approved prevention program focused on anger management and nonviolent conflict resolution. It includes the child’s agreement to participate, parental or guardian consent, and a required oath confirming compliance with legal restrictions.
