MAD-FCS-002 - COURT ORDER APPOINTING CHILD PROTECTIVE SERVICES IN PROBATE GUARDIANSHIP FOR NON-RELATIVE (Pursuant to Probate Code § 1513). This order appoints the Madera County Department of Child Protective Services (1610 Sunrise Ave., Madera, CA 93638) as the Court Investigator in non-relative probate guardianship matters under Probate Code §1513. It authorizes access to education records and grades; law-enforcement and parole/probation information; financial/banking and child-support records (including DCSS); social-services reports (CPS/APS); and medical, mental-health, and drug treatment/testing records consistent with HIPAA. The investigator must file a confidential report with the court and provide copies to counsel for the petitioner (or petitioner in pro per), counsel for the minor (if any), and any objector of record; further distribution requires a court order.
