FORM 25.6 – ORDER TO DISINTER REMAINS. This form is issued by the Ohio Probate Court under R.C. §§ 517.23–517.25 following approval of an Application for Order to Disinter Remains (Form 25.0). This court order formally authorizes the applicant to exhume and reinter a decedent’s remains after finding that all interested parties were properly notified or waived notice, and that disinterment does not conflict with the decedent’s religious beliefs. The order specifies the cemetery of original interment and the cemetery of reinterment, and whether a Board of Health permit was issued or filed as required under R.C. 517.23(B). It directs the applicant to file a Verification of Reinterment within 30 days after the remains are reinterred. The cemetery’s governing body—such as township trustees or cemetery association officials—has at least 30 days from receipt of the order to carry out the disinterment.
