Form G-05 - GUARDIAN'S INVENTORY FOR AN INCAPACITATED PERSON. This Orphans’ Court form is used by a guardian of the estate to report an incapacitated person’s assets, income, debts, insurance, care plan, and key medical directives to the Court of Common Pleas. Guardians indicate whether the filing is an initial or amended inventory, list co-owned property, identify insurance policies and safe-deposit boxes, and outline annual income sources and liabilities. The form also asks about surety bonds or professional-guardian coverage, housing and personal-care arrangements, government-benefit applications, and plans for maintaining or selling real estate. A verification page includes service requirements and penalties notice. Service of the Notice of Filing must occur within 10 days under Pa.R.O.C.P. 14.8(b); verification is subject to 18 Pa.C.S. §4904 (unsworn falsification). www.FormsWorkflow.com
