This is a Georgia Secretary of State Corporations Division form used when an out-of-state or foreign business authorized to transact in Georgia changes its entity name or home jurisdiction. Filers can also report changes to other information from the original application. The form captures the entity’s control number, type (corporation, LLC, LP/LLLP, LLP), new name or jurisdiction (if applicable), and desired effective date (upon filing or a delayed date within 90 days). A corporate filer with a new name must include a certificate of existence from its home state. The filing fee is $30 ($20 filing + $10 service charge).
