Description
DC-54 - COMPLAINT FOR EVICTION FOR NON-PAYMENT OF RENT. This form is filed by a landlord or property owner in the Rhode Island District Court to evict a tenant who is more than 15 days late on rent. This form initiates an eviction (possession) case under Rhode Island General Laws § 34-18-35, which governs residential landlord-tenant actions for non-payment. The form requires identifying information for both the landlord (plaintiff) and tenant (defendant), including addresses and contact details, as well as the rental property’s location and the judicial complex where the case is filed (Murray, Noel, McGrath, or Garrahy Judicial Complex). It also details the monthly rent amount, the total arrears owed, and the date the landlord sent the five-day demand notice, which must be attached to the complaint. The landlord must affirm that the tenant has not paid or offered full payment after receiving the demand notice and remains in possession of the property. The landlord asks the court for a judgment of possession (eviction), back rent, and reasonable rental value for continued use of the premises until judgment, plus court costs. The form must be signed and dated by the landlord or their attorney, including a Rhode Island Bar number if applicable. www.FormsWorkflow.com
