PR-12 - CONFIDENTIAL CONTACT INFORMATION FOR USE BY COURT INVESTIGATORS IN ACCORDANCE WITH PROBATE CODE §§ 2250.6(a)(1); 1826(a) and (l), 1850 and 1851(a) and (b). This form (completed) must accompany all petitions for appointment of probate conservators, successor conservators, or co-conservators (Limited or General, Temporary or Permanent). All references to conservatee or conservator in this form include proposed conservatee(s) and conservator(s). Additionally, this form (completed) must accompany all accountings filed by conservators of an estate. Attachments should be added as necessary to provide complete information. The Court must be notified immediately if the address of the conservator or conservatee changes. Form GC-079, or GC-080 must be used for the conservatee when a change in placement occurs.
