Delaware Supreme Court Rule 55.1. - Form 4 - CERTIFICATE HOLDER REPORT OF CHANGE IN CIRCUMSTANCES AND APPLICATION TO TRANSFER. This form is used by attorneys holding a Certificate of Limited Practice to report changes affecting their eligibility and, if applicable, request a transfer of the certificate to a new employer. (Part A) allows the certificate holder to report employment termination, loss of active bar status, disciplinary sanctions, or any other event impacting Rule 55.1 compliance. The form also requires the attorney to state whether they intend to transfer the certificate and acknowledges that failure to complete Part B within 90 days results in automatic termination. (Part B) collect information about the new employer and include a sworn certification confirming compliance with professional rules and Rule 55.1 requirements.
