JV-152 - WAIVER OF TEMPORARY CUSTODY HEARING G.L. c. 119, § 24 or § 25. This form is used in the Massachusetts Juvenile Court, allows a parent, guardian, or custodian to give up their right to a hearing under G.L. c. 119, § 24 or § 25 regarding temporary custody of their child(ren). By signing this form, the parent or guardian acknowledges that they understand they are waiving their right to be heard in court about whether their child(ren) should be removed from their custody, to challenge whether the Department of Children and Families made reasonable efforts to prevent the removal, and to appeal any determinations or orders made at that hearing. The form also includes the option to waive or reserve the right to nominate a temporary custodian to care for the child(ren).
