This form is used in the County or Circuit Court of St. Lucie County, Florida, by self-represented individuals (parties without an attorney) who are unable to comply with the requirement of receiving court documents by e-mail. Under Florida Rules of General Practice and Judicial Administration 2.516(b)(1)(D), a self-represented party may file this request if they do not have an e-mail account or do not have regular access to the internet. By submitting this form, the individual asks the court for permission to instead receive notices, orders, motions, pleadings, and other case-related documents by mail or personal delivery. The filer must provide their current mailing address and agree to keep the court and opposing parties updated on any changes. The request must be signed under penalty of perjury and served on the other parties.
