NHJB-2950-DFS - RECORDS RESEARCH SUBSEQUENT PAGE. This form is used by the New Hampshire Judicial Branch to request additional court record searches when the primary Records Research Request form does not provide enough space. It allows for up to 25 additional names (numbers 26–50) to be submitted for criminal, civil, or other case searches across the state’s circuit, superior, and family courts. Each entry requires the name, date of birth, and type of search (electronic records from 1992 forward or manual records prior to 1992). Requesters may indicate if they want results emailed or returned by mail, and searches cost $6.50 per name, per court.
