SC015 - SMALL CLAIMS EVIDENCE COVER SHEET. This form from the Superior Court of California, County of Marin, is used to organize and submit evidence in a small claims case. This form must be completed by either the plaintiff or defendant and attached to any documents being submitted as evidence for the court hearing. The party submitting the evidence must also serve a copy of the evidence and cover sheet to the opposing party—either by email or by having a non-party adult mail it. The form requires details about who is submitting the evidence, how and when it was sent to the other party, and includes a declaration under penalty of perjury.
