23 E-2 - CIVIL/FAMILY WARRANT APPLICATION. This form is used in the Circuit Court of the Twelfth Judicial Circuit, Will County, Illinois, to initiate the issuance of a civil or family-related warrant in accordance with Administrative Order No. 92-30. This form must be fully completed and submitted to the Will County Circuit Clerk’s Office, along with proof of payment for service fees from the Will County Sheriff’s Office for applicable case types (those beginning with L, LA, SC, EV, or LM). The form collects detailed identifying information about the individual for whom the warrant is being requested, including their name, physical description, address, date of birth, and driver’s license number. Optional fields allow for additional descriptive details such as scars, tattoos, or aliases to assist in locating and serving the individual. After submission, the Clerk prepares the warrant, the judge signs it, and it is forwarded to the Sheriff’s Warrant Division for processing.
