KRN SUP CRT PB-019 - REFERRAL FOR COURT INVESTIGATOR-CONSERVATORSHIP CHANGE OF ADDRESS (PROBATE – KERN COUNTY). This form is used in the Superior Court of California, County of Kern, to notify the court and request a court investigator’s review regarding a proposed change of address for a conservatee in a conservatorship case. It is used in probate matters where a (proposed) conservator is seeking to relocate a (proposed) conservatee, either temporarily or permanently. The form collects important contact information for both the conservator and the conservatee, details the current and new residence, and requires an explanation of the reasons for the move. Upon submission, it refers the matter to Family Court Services for investigation to ensure that the change of residence is appropriate and in the conservatee’s best interest.
