This form is used in the Superior Court of California, County of Kern, to notify the court and request a court investigator’s review regarding a proposed change of address for a conservatee in a conservatorship case. It is used in probate matters where a (proposed) conservator is seeking to relocate a (proposed) conservatee, either temporarily or permanently.
