This form is issued by the Missouri Secretary of State’s Office, Commissions Division, for non-resident notaries who have changed their place of employment to a new county in Missouri. This form allows the notary to request an amended commission reflecting the new employer county. Applicants must provide their name, date of birth, residence address, current commission number, new employer information, and effective employment date, along with a $5 application fee payable to the Secretary of State.
