AFFIDAVIT OF METES AND BOUNDS. This form is used in Champaign County, Illinois, to confirm that a deed being recorded does not violate Section 1 of Chapter 109 of the Illinois Revised Statutes, which governs the division and subdivision of land. This affidavit must be completed by the affiant—typically the property owner or grantor—who affirms that the attached deed qualifies for one of several exemptions under the law. These exemptions include land divisions involving parcels of five acres or more without new streets or easements; exchanges of land between adjacent property owners; conveyances for public utilities or transportation purposes; corrections of prior deed descriptions; or limited sales of smaller parcels under specific conditions. The affiant selects the applicable exemption from a numbered list of ten options and certifies its truthfulness under oath before a notary public.
