EXHIBIT FM-3 - NOTICE OF ENTRY RESIDENTIAL FORECLOSURE MEDIATION PROGRAM. This form is used by the Circuit Court of the 17th Judicial Circuit in Illinois to officially inform the parties involved in a residential foreclosure case that the matter has been submitted for potential mediation through the Winnebago County Residential Foreclosure Mediation Program. Once this notice is issued, all court proceedings in the case are stayed (paused) until the mediation process is resolved. The notice includes the names and addresses of the plaintiff and defendant, and it is signed by the Program Administrator.
