CV-11D - STATEMENT OF CONSENT TO PROCEED BEFORE A UNITED STATES MAGISTRATE JUDGE (For use in Reassignment of Civil Cases to United States Magistrate Judges on Voluntary Consent List only). This form is used in the United States District Court for the Central District of California when all parties in a civil case agree to have the matter reassigned to a Magistrate Judge from the court’s Voluntary Consent List, as authorized under General Order No. 24-06. By signing this form, the parties acknowledge that they voluntarily and jointly consent to have the selected Magistrate Judge handle all aspects of the case—including trial and final judgment—under the authority granted by 28 U.S.C. § 636(c) and Federal Rule of Civil Procedure 73(b). The form makes it clear that consent is optional and that withholding consent will have no adverse consequences. If consent is given and approved by both the assigned District Judge and the selected Magistrate Judge, the case is officially reassigned. Appeals from final decisions will go directly to the U.S. Court of Appeals, just as they would from a District Judge.
