Form 3-24 - PETITION FOR MODIFICATION OF ORDER OF COMMITMENT. This form is used in New York Family Court to request a change to an existing order of commitment for a juvenile delinquent who has been placed in a residential facility under the supervision of the New York State Office of Mental Health (OMH) or the Office for People with Developmental Disabilities (OPWDD). The petition may be filed by the Commissioner of either office or by another authorized representative on behalf of the respondent. It is used when continued commitment in a residential setting is no longer appropriate, and the petitioner seeks court authorization for the youth’s transfer to a non-residential facility or other appropriate placement.
