Form 358-a- 6 - PETITION FOR PRE-PLACEMENT APPROVAL OF STANDBY PLACEMENT INSTRUMENT. This form is used by a county Department of Social Services official to request Family Court approval of a standby placement instrument under Sections 358-a and 384-a of the New York Social Services Law. This petition applies when a parent or legal guardian with a progressively chronic or terminal illness anticipates that their child may need foster care within 18 months and executes a written instrument designating an agency (and optionally a specific foster parent) to assume custody upon the parent’s death, incapacity, or debilitation.
